Download and complete the DKV CARD application.

Print and sign the DKV CARD application (incl. company stamp).

Now complete the direct debit mandate online:

DKV CARD application direct debit mandate

To enable us to process your application further please print of the Direct Debit Mandate*, complete and sign it. Send the original (signed) form with your application to your DKV office. Direct debit mandate for UK (66 KB)

Once we have received, approved and processed your completed documents, we will send you your DKV CARD as soon as possible.

*please note when you want to pay in Euros we can only take payments by DD if you have a Euro account outside the UK.
